Welcome to the City of Burnside Home Support Program.

Funded by the Department of Health, Disability and Ageing through the Commonwealth Home Support Program (CHSP), the Home Support Program offers a range of services to older people living in the Eastern Metropolitan Region. These services include home support, transport assistance and social activities, all designed to help you live independently at home and stay engaged with your community.

At the Home Support Program, you are at the centre of everything we do. We respect your right to make your own decisions about the life you want to lead. Our aim is to work with you to achieve your personal goals, enhancing your health and wellbeing.

Whether you need assistance getting to appointments, connecting with others socially, or sharing the load with tasks around the house, we are here to support you.

Who is eligible for services?

My Aged Care is the single entry point for all Australian Government subsidised aged care services, including CHSP. To be eligible for CHSP services, you must meet one of the following criteria: 

  •  aged 65 years and over; or
  • Aboriginal or Torres Strait Islander and aged 50 years or over; or
  • homeless or at risk of homelessness and aged 50 years or over.

My Aged Care will assess your eligibility for various services and provide approval in the form of referral codes.

If you live in the Eastern Metropolitan Region, you can choose the City of Burnside as the provider of your services. Please provide us with your referral codes from My Aged Care.

You can make a referral directly to My Aged Care by either phoning them or completing a online referral form on their website. A doctor’s referral is not required. If you need assistance navigating the aged care system, an Aged Care Specialist Officer at Services Australia can help.

How can I access services?

To access services through the Commonwealth Home Support Programme you will need to contact My Aged Care on 1800 200 422.

My Aged Care is the entry point for all community aged care services. My Aged Care will assess which types of services you are eligible for and provide you with approvals for these services. Once you have the approvals, you can then choose the City of Burnside as the provider of your services.

The City of Burnside Home Support team can assist you to navigate your way through aged care services, including making a referral to My Aged Care.

If you require interpreting services, staff can assist you to access appropriately qualified, accredited interpreters with the Australian Government’s Translating and Interpreting Service, phone 131 450.
